EBM far from being evidence-based has become a bit of a joke. It's easier to hide ideological positions in systematic reviews because on the face of it it appears 'systematic' unless you delve into it in detail

Here's just one 'systematic review' that wasn't systematic at all (pg 7 onwards)- there are so many others... several by some of the same authors. Full of errors, misrepresentations, and misinterpretations, but hailed as gold-standard evidence. bmj.com/sites/default/files/…

Worth looking at Viner's other reviews also, where basic errors were found, and overinterpretation of evidence was made. Many have detailed this again and again, but hasn't changed these being hailed as superior forms of evidence & informing the UK response

e.g. the Viner review on impact of school closure doesn't delineate the impacts of the pandemic from school closure at all - when some of the studies it quotes suggest strongly that the impact comes from the pandemic on families - but this is just not considered.

Yet, these systematic reviews have influenced govt response, RCPCH thinking (of which Viner was former director), and even global paeds thinking. Because most people don't read beyond the results of the reviews, & don't pick up the errors. Including peer-review.

Systematic reviews are only valid if they are systematic (unbiased appraisal of study inclusion and quality), and if the information pulled out is accurate, and is then combined considering the quality & differences in design. Unfortunately, that's not what's happened at all.

An appeal to editors and peer-reviewers- I know this is hard, but it's vitally important to actually look at the source studies, and material - worth choosing peer-reviewers who know the area really well and can identify these errors. One cannot assume that all is above board.

It's unfortunate that I can often know what the conclusion of a systematic review will be without reading it- but just looking at the authors who wrote it. That shouldn't happen.

Your child’s pediatrician probably has never seen a case of polio, diphtheria, measles, congenital rubella, or even H. influenzae type b meningitis. We have vaccines to thank for that. Let’s not allow short memory spans and misinformation to take us back to a much darker time.
